Requirements

Applying is simple; make sure you follow all the requirements when you fill out the online application. A valid ID is required. You must also take a clear selfie with your ID or Driver’s License next to your face.

Use the upload feature on the form to upload all of your photos. In the subject line you can write in what you’d like your online screen name to be, and use the message area to write in alternative screen names.
